Direct Primary Care

Trying to decide if DPC is for you? We provide no-charge consultation with you (in person or on video) to discuss your healthcare needs.  We will review our membership services, fees, and discuss your needs to help ensure that our DPC model is the right fit for you.  If you decide to join our DPC, our initial visit is scheduled for 90 minutes in our office.  This visit will include a comprehensive physical exam, review of your health history, family history, as well as a lifestyle and diet review.  We will then set a schedule for regular check-in and follow-up visits throughout the year.  Visits can be conducted in the office, over the phone, or over video. 

Frequently Asked Questions

What if I have a problem and need to see you before my next scheduled appointment. Can I see you sooner?

Certainly! You are able to send messages 24/7 via our patient portal. You may also contact the office by phone or text.  Typically calls are returned within 8 hours on business days (and often sooner!). Appointments for urgent concerns will be offered same day or next day.  

Can DPC take the place of my health insurance?

No, DPC is NOT health insurance. We highly recommend patients have a health insurance plan to cover them for emergencies and services that are not covered under their DPC membership (such as visits with specialists/other healthcare providers, lab-work, imaging, and other diagnostic testing). Additionally, your health insurance may still be a resource to pay for testing and treatment that we cannot provide directly in our office (such as prescriptions from the pharmacy, lab-work, or referrals to specialists). 

Why would I pay for services at your practice if I have health insurance?

If we participate with your health insurer, you should not enter into a DPC agreement with our practice.  Your health insurer has already negotiated rates with our practice to provide healthcare services to you as one of their members.  However, for patients who have a health insurance plan that we are not in network with, DPC might offer accessible care, that actually costs them less in co-pays and deductible expenses.  


How much does a membership cost?

We will meet with you to discuss your healthcare needs and help you determine which level of membership if right for you.

Our Primary DPC membership meets most patients' primary care needs. Primary DPC patients pay $350 for their first month of service, and then $100/month thereafter.  Our standard monthly billing is done on the 1st of each month via automatic payments on credit or debit card, but we can work with you on a different payment arrangement if you prefer to pay with cash or check.

What is included in a DPC membership?


Office visits with our practice (including telehealth), prescription refills, telephone triage, and some basic tests are included in the cost of your membership.  We do not bill you for any of the services specified in your DPC membership agreement.  

What is not included in a DPC membership
?

Care outside our office is not covered under your DPC membership. Examples of care not included with your DPC membership are: Vaccinations, infused medications or fluids (IV, SQ, IM), visits with specialists or other healthcare providers, hospital-based encounters (surgery, ER visits, etc), lab-tests not processed and resulted in our office, costs of medications we prescribe for you at the pharmacy, etc.  

Do you coordinate care with my other healthcare providers and specialists
?

Yes! As your primary care provider, we will work to coordinate care with your specialists and make sure that your care plan is carefully summarized during your visits with our practice and available in your electronic medical record. Our MEDENT EHR, integrates with outside labs and medical practices through RHIO (Regional Health Information Organization), which serves to integrate care for patients accessing care with providers across our region. 

Can I cancel my membership? Can I return if I change my mind?

Yes and Yes! Patients may cancel at any-time with 30 days notice, any advance payments in excess of 30 days service will be issued by check and mailed to the patient within 2 weeks of their notice to terminate care. 
Patients who have cancelled their membership may request to reenroll in the practice in the future. Returning patients will be subject to the first-month fee before resuming their regular monthly fee.
